Terms of service

The terms governing your Blue Line Health membership.

Plain-English summary of what you agree to when you sign up, how billing and cancellation work, what the membership covers, and how we handle your records. Effective for members joining in 2026.

1. The membership at a glance

Blue Line Health is a direct-care membership program. By signing up you agree to pay a recurring monthly fee in exchange for the services described on our pricing page and in your signup confirmation. The membership is not health insurance, not a substitute for health insurance, and not a minimum essential coverage plan under the Affordable Care Act.

Your membership covers comprehensive virtual primary care, urgent sick visits, chronic-condition check-ins, routine labs, and prescription coordination, as described in our membership plan documentation. Hospitalizations, surgeries, emergency room visits, and most specialty care are not part of the base membership unless you have added the catastrophic coverage add-on.

2. Eligibility

You must be at least 18 years old to enter into this agreement. If you are signing up on behalf of a minor or another family member, you confirm you have the legal authority to do so. Blue Line Health is available to residents of the United States. We may decline to onboard a member who lives outside our service area or whose needs fall outside the scope of primary care.

3. Billing, renewals, and cancellation

Membership fees are billed monthly through our payment processor (Stripe). Early-member pricing is locked in for the first year of the practice; after that, renewal pricing reflects the membership tier at the time of renewal.

If you cancel within thirty (30) days of your first successful charge, we refund the full amount of that charge minus a nominal processing fee, processed within ten business days. Continued care coordination, medication refills, and a copy of your clinical records remain available to you after cancellation.

After the 30-day window above, the membership is month-to-month — cancellation takes effect at the end of the current billing period, and we do not prorate partial months outside that window. You can cancel at any time from your member dashboard or by emailing us.

If your payment method fails, we will retry and notify you. After a reasonable grace period, your membership will be paused until payment is corrected. A paused membership loses access to live scheduling, text-a-physician, and cash-price routing.

Add-ons (such as the catastrophic coverage add-on) are billed the same way on top of the base membership, monthly through Stripe. You can view, add, or cancel add-ons from your member dashboard at any time. Adding an add-on mid-cycle is prorated to the remainder of the current billing period; the next full month kicks in on the next billing date. Canceling an add-on takes effect at the end of the current billing period, and any mid-cycle add-on removal produces a pro-rated credit applied to your next charge — never a cash refund. You cannot add an add-on without an active base membership, and canceling or pausing your base membership automatically cancels every add-on attached to it. Add-on credits do not stack with the base membership’s month-to-month rule: a credit is consumed by the next base-membership charge and is not paid out in cash.

4. Refunds

Continued care coordination, medication refills, and a copy of your clinical records remain available to you after cancellation, regardless of how the cancellation was processed.

5. What you can expect from us

Your Blue Line Health virtual primary-care physician commits to: prompt responses to text messages (typically within minutes during clinical hours); same-day or next-business-day virtual visits for acute issues; annual wellness check-ins on each renewal; transparent routing to in-network or cash-pay options with prices quoted up front; and continuity of care coordination if you are hospitalized or seen by a specialist.

Our clinical team operates within the license scope, training, and professional standards that apply to their state. We do not prescribe controlled substances outside what is clinically defensible, and we do not provide care that falls outside primary care without an explicit referral.

6. What we ask of you

Treat our team with the same respect you would expect. Provide accurate medical history and current medications. Show up on time for scheduled visits or give reasonable notice. Understand that urgent message routing is for non-emergency clinical questions — for true emergencies, call 911 or go to the nearest emergency room.

7. Privacy and your records

Your health information is governed by our Privacy Policy. In short: we protect your records with industry-standard safeguards, we do not sell your data, and we share information only with you and with the providers you have explicitly authorized.

8. Telehealth and asynchronous messaging

Most day-to-day care is delivered by text, secure message, or video visit. Telehealth services are subject to the laws of the state where you (the patient) are physically located at the time of the visit. If you travel across state lines, plan a brief notification so we can confirm coverage under that state’s licensing rules.

9. Disclaimers and limits of liability

Blue Line Health provides primary care services only. We do not warrant that a specific clinical outcome will result from any visit, prescription, or referral.

Suggestions from our AI patient navigator — including any draft messages, follow-up prompts, and routing recommendations it produces — are not medical advice. They are tools that assist your clinician; clinical decisions and prescriptions are made by your licensed physician within their state’s scope of practice.

To the maximum extent permitted by law, our total liability for any claim arising from your membership is limited to the amount of membership fees you have paid in the twelve months immediately preceding the event giving rise to the claim.

Nothing in these terms is intended to override mandatory state or federal consumer protection laws that apply to you.

10. Changes to these terms

If we make material changes to these terms, we will email every active member at least 30 days before the changes take effect and post the updated terms on this page. Continued use of your membership after the effective date constitutes acceptance of the updated terms.

11. Medical Disclaimers

Blue Line Health delivers care exclusively via telehealth. We do not see patients in person at any physical location; if you need hands-on examination, imaging, or in-person treatment, your physician will refer you to an appropriate in-person provider.

Our 24/7 urgent-care line is for non-emergency clinical questions that can safely wait for the next available clinician. For true emergencies — chest pain, difficulty breathing, signs of stroke, severe bleeding, suicidal thoughts, or any other life-threatening situation — call 911 or go to the nearest emergency room immediately.

We do not prescribe DEA-scheduled controlled substances. If your clinician determines that a scheduled medication is clinically indicated, they will refer you to an appropriate prescriber who can evaluate and, when appropriate, issue the prescription in compliance with DEA rules.

Our AI patient navigator is informational. It summarizes your record, drafts messages, and surfaces routing recommendations for your clinician’s review. Use of the AI navigator on its own does not establish a clinician-patient relationship; that relationship is formed only when a licensed Blue Line Health physician assumes clinical responsibility for your care.

Pricing and fee disclosures on the website, in signup materials, and in member communications describe our charges. They are not a guarantee that any service, prescription, lab, or referral will be covered by your health-insurance plan, employer benefit, or any third-party payer.
